Del Sur CSC Waiver of Responsibility Form
ACTIVITY DESCRIPTION
I am aware that the participation in Del Sur CSC Snow Day activities, events, tournaments,
games, practices and/or programs has, in addition to the usual dangers and risks inherent in
winter sledding, certain additional dangers and risks including, but not limited to, the danger
and risk of collision with other participants and/or man-made objects (sleds, jumps, signs), and
I freely accept and fully assume all such dangers and risks and the possibility of personal injury,
death, property damage or loss resulting therefrom.
ACKNOWLEDGEMENT OF RISK
I am aware that playing or practicing to play/participate in any sport or outdoor activity can be
a dangerous activity involving MANY RISKS OF INJURY. I understand that the dangers and
risks of playing or practicing to play/participate in an event such as the Del Sur CSC Snow Day
(Planned by MR Events) include, but are not limited to, death, serious neck and spinal injuries
which may result in complete or partial paralysis, brain damage, serious injury to virtually all
internal organs, serious injury to virtually all bones, joints, ligaments, muscles, tendons and
other aspects of the muscular skeletal system, and Serious injury or impairment to other
aspects of my body, general health and well-being. I understand that the dangers and risks of
playing or practicing to play/participate in an event like the Del Sur CSC Snow Day (Planned by
MR Events) may result not only in serious injury, but in a serious impairment of my future
abilities to earn a living, to engage in other business, social and recreational activities, and
generally to enjoy life.
DISCLOSURE
Having acknowledged the existence of the above risks to my person that may arise out of or in
connection with my taking part in sledding at The Village Green Park and participating in the
Del Sur CSC Snow Day, I hereby declare and acknowledge that I have the following physical
and/or medical condition(s) and/or infirmities which may impact upon the Del Sur CSC Snow
Day organizer’s and Meaghan Renee Events decision to permit me to take part in said event
and which may affect my ability to successfully take part in said event of which may be
significant in case of emergency or accident. These physical and/or medical condition(s) are:

INDEMNIFICATION AND RELEASE
In signing this waiver/participation form, I elect to use the facilities and equipment provided by
Snow Day event organizers, Meaghan Renee Events and the Del Sur CSC, and participate in
the Del Sur CSC Snow Day at my own risk, and do thereby release the Executive Members of
the Del Sur CSC planning team and Meaghan Renee Events together with their heirs, assigns,
officers, representatives, agents, employees and members, sponsoring organization and
owners of properties or facilities in or on which the competition is to be held, from all liability
from injury to person, property, and/or reputation that I may receive and from all claims of said
injuries growing out of, or resulting from event participation, or caused by any construction or
condition of the facilities or equipment used in the event. By signing this waiver, I also
acknowledge that I am 18 years old or older, or that my legal guardian has co-signed with me.
I have read the supplementary regulations issued for the Del Sur CSC Snow Day and agree to
be bound by them. In signing this document, I agree that my sled meets all requirements and
that I will not hold any event organizer or presenting entity responsible for my workmanship. In
consideration of the Del Sur CSC, Meaghan Renee Event organizers, presenting partners and
event sponsors, agreeing to permit my participation in the Del Sur CSC Snow Day, I AGREE TO
SAVE HARMLESS AND KEEP INDEMNIFIED the Del Sur CSC, Meaghan Renee Events, event
organizers, and all event sponsors and presenting partners, its organizers, executives and their
respective agents, officials, employees and representatives from and against any and all
claims, actions, costs and expenses and demands with respect to death, injury, loss or
damage to my person or property, including but not limited to those damages set out in the
acknowledgment of risk, howsoever caused, arising out of or in connection with my taking part
in the Del Sur CSC Snow Day event and notwithstanding that the same may have been
contributed to or occasioned by the negligence of the said bodies, or any of them, their agents,
officials, employees, or representatives. It is understood and agreed that this agreement is to
be binding upon myself, my heirs, executors and assigns.
